  On-line Warranty Request Form : (receipt required)

2) By FAX
  24 Hours Available. Required Adobe Reader (Click HERE for Download Adobe Reader)
 

1. Print out the Warranty Request Form and fill it in completely.
2. Please fax the Warranty Request Form to 847-813-6305.        
3. The Hyundai Service Center will issue and send you a RMA Number.
4. If you do not have access to a facsimile machine, please call the Hyundai Service Center
       at 800-568-0060 (or 847-813-6300) on Monday through Friday from 9:00 AM to 5:00 PM
       for other instructions.

BRAND NEW EXCHANGE ADVANCE UNIT (Advanced DOA)
 
   
 
   

If your unit is Dead On Arrival (DOA) or WITHIN 30 days of purchasing it, you are eligible to participate in the Advance Shipment Policy program.

   1 . Print out the Warranty Request Form & Advance Shipment Billing Agreement and
       fill it in completely. Please note that you will NOT be billed for the Advance Shipment unless you
       fail to return the defective unit, as described, within 30 days.
   2. Please fax the following three documents to 847-813-6305:

           Completed Warranty Request Form
           
Completed Advance Shipment Billing Agreement
          
 A good, clear copy of the original purchase receipt

   3. The Hyundai Service Center will then advance ship a new monitor to you.
   4. Please note: you must return the defective unit and all accessories to avoid being billed.
   5. If you do not have access to a facsimile machine, please call the Hyundai Service Center
       at 800-568-0060 (or 847-813-6300) on Monday through Friday from 9:00 AM to 5:00 PM
       for other instructions.
